All information collated is done so in order to aid understanding of your level of health, fitness and ability to ensure any treatment received by yourself is relevant to your requirements and is done so in a safe manner. It is essential that the information gathered is relevant and correct to the best of your knowledge. However, it is your personal choice what information you choose not to share with myself. If specific information is required, but you are unwilling to divulge it, please understand that treatment may cease immediately.

There are circumstances where information will need to be shared, for example, when communication is needed between myself and another health professional whom you may be receiving treatment and/or advice from. If this is the case, your consent will be sought before contact is made with said health professional.
Your data is stored by Rebecca Stone BSc (Hons) Sports Therapy in both paper form and via computer. All paper-work is filed away and secured under lock and key and computer devices which may provide access to your information are protected by password in accordance with the Data Protection Act (1998) and GDPR (2018).
Patients of Rebecca Stone BSc (Hons) Sports Therapy have the lawful right to submit a ‘subject access request’. This may only be done so through direct contact with myself and I require this request to be put in writing. It is true that the general public have the right to request their personal information be deleted and destroyed. However, please be aware that the requirement for Rebecca Stone BSc (Hons) Sports Therapy to retain your information for up to 10 years following the termination of our professional relationship (for insurance purposes) supersedes the request for information to be deleted.
